Step to getting set up with a provider.
1. Once you have filled out our communication form we will reach out to you via a phone call and/or an email.
2. We will do a brief screening and collect insurance information to assess if we can meet your needs. If we can't met your needs our staff will help you find a provider who will.
3. If you would prefer to have a 15 min meet and greet please ask and we can schedule a phone call, video call or in person meeting.
4. You will create a Simple Practice account (HIPPA compliant electronic health record system) and complete our intake paperwork.
5. You will meet with your therapist for an intake assessment and to get started.
