Becoming a New Client

Step to getting set up with a provider. 

1. Once you have filled out our communication form we will reach out to you via a phone call and/or an email. 

2. We will do a brief screening and collect insurance information to assess if we can meet your needs. If we can't met your needs our staff will help you find a provider who will. 

3. If you would prefer to have a 15 min meet and greet please ask and we can schedule a phone call, video call or in person meeting. 

4. Create a Simple Practice account (HIPPA compliant electronic health record system) and complete our intake paperwork. 

5. Meet with your therapist for an intake assessment to discuss what you would like to work on and get started.
